IMMUNIZATIONS

A record of immunizations is due at the time of application/registration. This record will be reviewed by the school nurse and checked for compliance to New York State guidelines. Please present a new record when additional vaccines are administered at the doctor's office.

HEALTH CERTIFICATE/ APPRAISAL FORM/PHYSICAL FORM

This is required for all students who are entering SSPJ for the first time as well as for students entering

kindergarten, Grade 2, Grade 4, Grades 6, 7 & 8

This is recorded on a Smithtown Central School District form. It must be dated no more than one day after the first day of school the previous year and submitted to the Health Office by September 30". If a physical form is not submitted an appointment will be made with the school doctor.

DENTAL CERTIFICATE

This is to be completed by the dentist for all first time students as well as for students entering

kindergarten, Grade 2, Grade 4 and Grade 7

SPORTS PARTICIPATION MIDDLE SCHOOL

A HEALTH HISTORY FORM is required to be complete by the parent. The purpose of this form is to have a complete assessment of your child so that both your child's doctor and the school doctor can ascertain whether or not your child can safely participate. Please make sure you complete it and show it to the doctor prior to having the Health Certificate filled out.

If the report of your child's Health Certificate is more than 30 days old prior to the start of the sports season, the school nurse will ask you to complete an MEDICAL UPDATE FORM. This form will need to be signed by a parent and the student athlete.

SCHOOL HEALTH SCREENINGS

All students will receive vision, hearing and scoliosis screening according to NYS guidelines. Please see the overview grid in the document section to determine whether your child’s class will be screened this school year.

MEDICATIONS

If your child requires regular, over-the-counter or emergency medication in school, please contact the school nurse to have the required forms completed.
